Social Security Numbers on Work Permits
A B1-1 Form (Statement of Intent to Employ a Minor and Request for a Work Permit (Work Permit Application) must be fully filled out before a work permit can be processed (EC 49110) and that Intent to Employ form (B1-1) must have an SSN (EC 49163) on it. In California, the B1-1 serves a double purpose in that it is the “application” for the work permit and it also serves as the state’s version of the Certificate of Age (Title 5, California Code of Regulations, § 10120.1) which is a federal requirement of every state by the Fair Labor Standards Act (work permits are a state-level document only). The B1-4 must be produced with the SSN (EC 49115) Remember, it is also a legal document.
A reminder, CDE controls the forms B1-1 and B1-4 and no modifications are allowed (EC 49117, EC 49162).
