Athletic Clearance

Every student wishing to participate in athletic activities -- summer activities, pre-season conditioning, tryouts, practices, or contests -- must be cleared by GBHS Athletics each school year. As a part of the clearance process, every athlete is required to have a yearly physical from a qualified physician one time per school year and register for athletic clearance.

Sports physicals must be dated on or after May 19, 2025. 

Clearance Process

  1. Register for an account through homecampus – using the parent/ guardian email address.
  2. Get your athletes' STUDENT ID # from them or your Homelink account.
    • If you have an incoming freshman and do not have access to your Homelink account, please enter "000000".
  3. Complete information input on homecampus.  
    • You must have insurance or purchase school insurance in order for your student-athlete to be cleared
  4. Upload your completed Sports Physical Form
    • Signed and stamped by an MD, DO, NP or PA only, (BOTH pages) to the site toward the bottom of the student information page.  
    • Please do not email, mail or drop off forms to the Athletic Secretary.  No paper copies are accepted.  Coaches will NOT collect any physicals.  Make sure the athlete's name is on the forms.
  5. Add all sports that the athlete would like to participate in during the whole school year.

Disclaimers

  • The sports physical form MUST be stamped, signed and dated by the Doctor's Office (MD, DO, NP or PA only), no business cards are accepted, no exceptions.
  • Your athletes' account will be reviewed manually.
    • This can take up to 48-72 school business hours, as all clearances are done manually. 
  • You will receive an email stating that your child has been CLEARED by GBHS Athletics or Denied, if you are missing items. This can take up to 48-72 school business hours, as all clearances are checked manually. Coaches will check clearances before the athlete may participate.
  • **ONLY 1 item must be uploaded to Athletic clearance account
  • Sports Physical (2 pages - signed and stamped)-  upload to Physical Form area only.
  • Please note that if your student plays multiple sports, you will be required to register for those sports as well.  All sports will be open to sign-up during account registration.  If you do not sign-up for a particular sport, the student-athlete will not show up on the "cleared" roster for the coach and will not be allowed to participate. If you need to add a sport, the parent/guardian must go into account and add sport to the account.
  • Athletes are required to have a tetanus booster within the last ten years preceding participation. Student athletes with authorized medical reasons may be exempt from this requirement. 

Transfers

Transferring from one school to another school may affect a student's athletic eligibility under CIF rules.  It is your responsibility to see the Athletic Director or Athletic Secretary for transfer paperwork.  All eligibility transfer information/ applications MUST be true, correct, accurate, complete and/or not false or fraudulent.  Go to www.cifsjs.org for further information on Transfer Eligibility.
